Mentor Registration Form

Complete the form below to register to become a UB Career Mentor. It will be emailed to the Career Services office. You will receive a confirmation letter and password to access your Mentor record online for viewing and editing. Your confirmation will be emailed if you provide an email address in your registration form.

If you wish to withhold any information for release on the web to UB students/alumni registered with our office (password protected site), place a check in the box where indicated.
